You eat the same, you move about the same, and your cholesterol results have got worse. This is one of the most consistent metabolic changes of the transition, it happens to women who have done nothing differently, and it is the reason cardiovascular risk rises after menopause rather than gradually with age.

What actually changes

Studies following women through the transition find a consistent pattern, with the sharpest shift in the year or two around the final period:

  • LDL cholesterol rises — the fraction most closely tied to cardiovascular risk
  • Total cholesterol rises
  • Triglycerides tend to rise
  • HDL may rise slightly, but its protective function appears to change, so a "good" HDL number means less than it used to
  • Lipoprotein(a) — a largely genetic risk factor, measured far too rarely — can also increase

The important nuance: this is not simply ageing. Research separating chronological age from menopausal stage finds the lipid shift tracks the menopause transition itself, which is why it can appear over two or three years rather than creeping up over decades.

Why it happens

Estrogen influences how the liver handles cholesterol, including the activity of LDL receptors that clear LDL from the blood. As estrogen falls, clearance becomes less efficient.

So the message worth internalising is that your results did not get worse because you got lazy.

What to get measured

A standard lipid panel gives total cholesterol, LDL, HDL and triglycerides. Worth asking about in addition:

  • Non-HDL cholesterol — total minus HDL. Many guidelines now prefer it, and it does not require fasting
  • Lipoprotein(a), once in your life. It is largely genetic, it is not modified much by lifestyle, and knowing it changes how aggressively everything else is managed. Most women are never offered it, and a single test is enough
  • ApoB, where available, which some clinicians consider a better measure of risk than LDL alone
  • HbA1c and blood pressure at the same visit, because they are managed together — see blood pressure and menopause

Reading the result properly

A single number out of range is not the point. Treatment decisions are made on overall cardiovascular risk, estimated from your lipids together with age, blood pressure, smoking, diabetes, family history and other factors.

Two things women are rarely asked about, which genuinely change the estimate:

  • Pre-eclampsia or gestational diabetes in any pregnancy
  • Early menopause, before 45, which is itself associated with higher cardiovascular risk

It is also worth knowing that risk calculators developed largely in male populations have been criticised for underestimating women's risk, and that women's cardiac risk is documented as under-recognised and under-treated. That is context for asking questions rather than accepting a shrug.

What actually moves the numbers

  • Replace saturated fat rather than just cutting it. Swapping toward unsaturated fats — olive oil, nuts, oily fish — lowers LDL more reliably than removing fat and eating more refined carbohydrate
  • Soluble fibre. Oats, barley, beans, lentils, psyllium. One of the few dietary changes with a direct, measurable LDL effect
  • Lose visceral fat if you carry it. Waist measurement tracks this better than weight
  • Exercise. Aerobic activity chiefly improves triglycerides and HDL; resistance training supports the metabolic picture — see strength training in menopause
  • Reduce alcohol, which raises triglycerides directly — see alcohol in midlife

  • Plant stanols and sterols have modest evidence for lowering LDL
  • Treat sleep apnea if present — see sleep apnea after menopause

Realistic expectation: lifestyle change typically moves LDL by a modest percentage. That is worthwhile and it is often not enough on its own if your risk is high or your starting point is very elevated. Both halves of that sentence matter.

Does HRT lower cholesterol?

It does affect lipids — oral estrogen in particular tends to lower LDL and raise HDL, while also tending to raise triglycerides. Transdermal estrogen has less effect on lipids overall.

But this is important: hormone therapy is not approved or recommended for cardiovascular prevention, and should not be started to improve cholesterol. Favourable movement in a lipid panel is not the same as demonstrated reduction in heart attacks, and that distinction is the core lesson of the last twenty-five years of research in this area.

What is reasonable: if you are taking or considering hormone therapy for symptoms, your lipid profile is part of choosing the route. High triglycerides, for instance, are a reason transdermal may be preferred. See HRT types and forms and HRT risks and benefits.

Statins and the conversation to have

If your risk is high enough that medication is suggested, it is worth engaging with it rather than reflexively declining.

Reasonable questions:

  • "What is my estimated ten-year risk, and what would treatment change it to?"
  • "Does my family history or Lp(a) change that estimate?"
  • "What are the realistic side effects, and what do we do if I get them?"
  • "What would we expect from three to six months of lifestyle change first?"

Muscle aches are the most commonly reported side effect and the most common reason people stop. Worth knowing: aches are also extremely common in midlife women anyway, from the transition itself — so a trial period, or a different statin, is often more useful than concluding you cannot tolerate them. What not to buy

Direct-to-consumer "cardiac panels," expensive supplements marketed for cholesterol, and red yeast rice — which contains a compound chemically related to a statin, at unregulated and variable doses, with the same potential side effects and none of the quality control. If you would consider that, consider the regulated version instead.
