Two supplements are genuinely worth considering around menopause, and both are routinely taken at the wrong dose, in the wrong form, or without the one thing that makes them work. This is a plain guide to what the evidence supports, what it does not, and the questions to ask so you are not guessing.

Why these two, and not the rest

Most supplements marketed at menopausal women have weak evidence — our review is at what the supplement research actually says. Vitamin D and calcium are different for a specific reason: bone loss accelerates around menopause, and these are the raw materials and the regulator of the system that maintains bone.

That said, they are not a treatment for osteoporosis on their own, and taking them does not substitute for the thing with the strongest evidence: resistance and impact exercise. See bone health in menopause.

Calcium: food first, supplement only to close the gap

Recommended intake for women over 50 is commonly cited as around 1,200 mg a day, from all sources combined.

The important part, and the part most often missed: that figure includes food. If you eat dairy, fortified plant milks, tinned fish with bones, tofu set with calcium, or leafy greens, you may already be close. Supplementing on top of an adequate diet does not add benefit and may add risk.

Rough guide to what a serving provides:

  • A glass of milk or a fortified plant milk: around 300 mg
  • A pot of yoghurt: around 200–300 mg
  • A matchbox of hard cheese: around 200 mg
  • Tinned sardines with bones: around 300 mg
  • A serving of calcium-set tofu, or fortified cereal: 150–300 mg

Add up a typical day. If you are landing near 1,000 mg, a supplement is probably unnecessary. If you are at 400 mg and do not eat dairy, a modest supplement makes sense to close the gap rather than to duplicate it.

Two practical points if you do supplement: split the dose, because absorption is better at 500 mg or less at a time; and take calcium citrate rather than carbonate if you take acid-reducing medication, since carbonate needs stomach acid to absorb.

On the risk question: some studies have raised concerns about calcium supplements and cardiovascular risk, while others have not, and the evidence is genuinely unsettled. Calcium from food has not raised the same questions. That is the practical argument for food first — not alarm, just a reason not to take more than you need.

Vitamin D: the one worth testing

Common recommendations sit around 600–800 IU a day for adults in this age range, with higher intakes sometimes advised for people who are deficient or at risk.

Who is likely to be low: people with limited sun exposure, darker skin, who cover up, who are older, who have higher body weight, or who have conditions affecting absorption such as celiac or inflammatory bowel disease. Winter at higher latitudes affects almost everyone.

This is the one where a blood test genuinely helps, because the right dose depends on where you start and it is one of the few supplement questions with a clear answer. Ask for a 25-hydroxyvitamin D level, particularly if you have risk factors or bone concerns. Our list is in health screening in your 50s.

Do not take very high doses without supervision. Vitamin D is fat-soluble and accumulates; excess causes high blood calcium, with nausea, weakness, kidney problems, and confusion. Very high intermittent doses have been associated in some trials with increased falls, not fewer. More is not better here.

The part everyone skips: exercise

Neither supplement builds bone. Loading does.

  • Resistance training twice a week — the intervention with the best evidence for bone and muscle in midlife; see strength training in menopause
  • Impact where you are able — hopping, skipping, step-downs. Walking maintains, but it does not load bone the way impact does
  • Balance work, because most fractures come from falls

If you take the supplements and skip this, you have bought the bricks and skipped the builder.

What else affects bone

  • Protein, which is structural in bone as well as muscle, and which most women under-eat at breakfast; see eating for menopause
  • Smoking, which is directly harmful to bone
  • Alcohol at higher intakes; see alcohol in midlife
  • Being underweight, or chronic under-eating
  • Some medications, particularly long-term steroids and certain acid reducers
  • Vitamin K2 and magnesium, which are often marketed alongside calcium — the evidence for adding them specifically for bone is weaker than the marketing implies

Where hormone therapy fits

Hormone therapy reduces bone loss and fracture risk, which is one of its established benefits. It is not usually prescribed for bone alone in women who have no symptoms, but if you are weighing it for hot flashes and also have bone concerns, that belongs in the calculation. If you already have osteoporosis

Supplements are supportive, not the treatment. Specific bone medications exist with strong fracture-reduction evidence, and if a DEXA scan has shown osteoporosis, that is the conversation to have — not a better calcium tablet.

One practical thing to remember: if you are prescribed a bone medication, tell your dentist before any extraction or implant, because sequencing matters. See menopause, teeth and gums.

What to ask

  • Could we check my vitamin D level, given my risk factors?
  • Based on what I eat, do I actually need a calcium supplement — and if so, how much?
  • Do my risk factors warrant a DEXA scan? (Early or surgical menopause, long-term steroids, a parent who fractured a hip, low body weight, smoking, heavy drinking, rheumatoid arthritis, or a fracture from a minor fall all count.)
  • Are any of my current medications affecting my bone?

Get your vitamin D tested and correct it if low. Add up your dietary calcium before buying any, and use a supplement only to close a real gap. Then do the resistance training, because that is the part that actually builds bone — and it is free.
