Sudden severe pain in the back or side that comes in waves, often with nausea and blood in the urine. Kidney stone rates have risen in women, they become more common with age, and several of the contributing factors — dehydration from night sweats, calcium supplements taken for bone — sit right in the middle of midlife health advice.

What an attack feels like

  • Severe pain in the flank or back, often radiating to the lower abdomen and groin
  • Waves rather than constant — classically described as worse than childbirth
  • Restlessness, unable to find a comfortable position. This distinguishes it from peritonitis, where people lie still
  • Nausea and vomiting
  • Blood in the urine, visible or found on testing
  • Urgency and frequency as the stone moves down

Seek urgent care for: severe pain, fever with flank pain (an infected obstructed kidney is an emergency), inability to pass urine, or persistent vomiting.

Why risk rises in this decade

  • Age, generally
  • Dehydration. The single largest modifiable factor — and night sweats, hot weather, and reduced fluid intake all contribute; see night sweats in perimenopause and getting through summer
  • Metabolic changes — obesity, insulin resistance and metabolic syndrome are associated with stone risk, and all shift after menopause; see insulin resistance in menopause
  • Diabetes — see menopause with diabetes
  • Gout and high uric acid, which rise in women after menopause and are associated with uric acid stones; see gout in women after menopause
  • Bone loss. Calcium mobilised from bone can raise urinary calcium
  • Certain medications, including some diuretics and topiramate
  • Family history

The calcium supplement point

Worth handling carefully, because the advice can seem contradictory.

Dietary calcium does not increase stone risk — it reduces it. Calcium in food binds oxalate in the gut so less is absorbed and excreted. Cutting dietary calcium to prevent stones makes them more likely, and this is one of the most persistent misconceptions in the area.

Calcium supplements are a different question. Taken between meals, they raise urinary calcium without the gut-binding benefit, and have been associated with stone risk in some research.

The practical resolution:

  • Get calcium from food where possible
  • If you take a supplement, take it with meals, which reduces the risk and improves the oxalate binding
  • Supplement to fill a measured gap, not by default

If you have had a stone, tell whoever advises you about bone health — it changes the calcium and vitamin D conversation, and it is a detail that falls between specialties.

Prevention that works

Fluid, above everything. The target usually given is enough to produce around 2 to 2.5 litres of urine a day — which means drinking more than that. Practically: urine should be pale straw, not dark.

This is the intervention with the strongest evidence, and it is free.

Reduce salt. High sodium increases urinary calcium. Most salt comes from bread, processed meat, sauces and eating out rather than the salt cellar — see blood pressure and menopause.

Keep dietary calcium adequate, as above.

Moderate animal protein, particularly red meat, which raises uric acid and urinary calcium.

Oxalate, if you are a calcium oxalate stone former — spinach, rhubarb, beetroot, nuts, and chocolate are high. Do not eliminate them blindly; pair them with calcium-containing food at the same meal, which binds oxalate in the gut.

Citrate, which inhibits stone formation. Lemon juice in water is the cheap version; prescription potassium citrate is used in some cases.

Weight management, and treating metabolic syndrome.

Vitamin C in high doses (above about 1,000mg daily) converts to oxalate and has been associated with stone risk. Not a reason to avoid dietary vitamin C.

Find out what kind of stone

This changes the prevention advice, and it is frequently skipped.

Strain your urine and keep the stone if you pass one — that is what allows analysis. Most are calcium oxalate; uric acid, struvite and cystine stones are managed differently.

After a stone, ask about: stone analysis, blood tests (calcium, uric acid, kidney function), and in recurrent cases a 24-hour urine collection to identify what is driving it.

Recurrence is common without prevention, which is why this matters.

Does HRT affect stone risk?

Research findings have been mixed, with some studies reporting an association between hormone therapy and stone risk and others not. It is not established, and it is not a reason to avoid hormone therapy.

What matters more, and is under your control: hydration. If you are having frequent night sweats, you are losing fluid and often not replacing it — treating the vasomotor symptoms and drinking more both address that. See HRT risks and benefits.

What else that pain might be

Flank pain has other causes worth distinguishing:

  • Kidney infection — flank pain with fever and urinary symptoms. Urgent; see recurrent UTIs after menopause
  • Musculoskeletal back pain, which is usually positional rather than colicky — see back pain in midlife
  • Gallbladder problems, typically upper right and after fatty meals — see gallbladder, liver and HRT
  • Gynaecological causes, including ovarian cysts and torsion — see ovarian cysts in perimenopause
  • A ruptured aortic aneurysm, rare and life-threatening, which is one reason severe new flank pain is assessed rather than managed at home

What to ask after a stone

"Could my stone be analysed, and could I have calcium, uric acid and kidney function checked? Given that I'm postmenopausal and taking calcium for my bones, should I change how or when I take it? And should I have a 24-hour urine collection?"
