You are told to take calcium for your bones, and then you read that calcium supplements might raise cardiovascular risk. Both messages come from reasonable sources, the disagreement is real, and there is a practical resolution that most coverage skips.

The short version

Calcium from food is not implicated. The cardiovascular question concerns supplements, particularly high doses taken without a dietary shortfall.

The evidence is genuinely mixed. Some analyses have found an association between calcium supplements and cardiovascular events; others have not. Major bodies have reviewed it and generally concluded that calcium intake up to the recommended level, from food and supplements combined, is not associated with cardiovascular harm.

The practical resolution: get calcium from food where you can, and use supplements to fill a measured gap rather than as a default.

That sentence resolves most of the anxiety this topic generates.

Why the concern arose

The proposed mechanism is that a supplement delivers a bolus — a sudden rise in blood calcium — in a way that food spread across a meal does not, and that this may affect vascular calcification.

Plausible, and not established. What is clearer is that more is not better: exceeding the recommended intake has not been shown to benefit bone further, and that is where the risk questions concentrate.

How much you need

Recommendations differ by country and by age, commonly falling in the region of 1,000 to 1,200mg a day for women in midlife and after menopause, from all sources combined.

Combined is the word that matters. If you eat dairy and fortified foods and then add a 1,000mg tablet, you may be well above the recommendation — which is the scenario the concern is about.

Working out what you already get

Approximate content:

  • 200ml milk — about 240mg
  • 150g yoghurt — about 200mg
  • 30g hard cheese — about 200mg
  • Fortified plant milk — often around 240mg per 200ml, if fortified; check, and shake the carton, because calcium settles
  • Tinned sardines with bones, 100g — about 380mg
  • Tofu set with calcium, 100g — often 200–350mg; check the label
  • 100g kale or broccoli — about 40–150mg
  • A slice of fortified bread — varies
  • Tinned salmon with bones — high
  • Almonds, 30g — about 75mg

Add up a typical day. Most women who eat some dairy or fortified plant milk are closer to the target than they assume.

Then supplement only the gap, in the smallest dose that closes it — often 500mg or less, rather than the 1,000mg tablets sold by default.

Practical points if you do take one

  • Split doses. Absorption of calcium is better in amounts of around 500mg or less at a time
  • Take it with food, which improves absorption of calcium carbonate in particular
  • Calcium citrate does not require stomach acid and is the better choice if you take a proton pump inhibitor or have low stomach acid
  • Separate it from levothyroxine by four hours, and from iron and some antibiotics by a couple of hours — see HRT and thyroid medication and how to actually take iron

  • Kidney stones, hyperparathyroidism, or a history of high blood calcium — speak to a clinician before supplementing at all

Vitamin D is the other half

Calcium is poorly used without adequate vitamin D, and deficiency is common — particularly with limited sun exposure, darker skin, or covering up.

Vitamin D is usually the more useful thing to test and correct, and it is cheap. See vitamin D and calcium.

Vitamin K2 is often sold alongside on the basis that it directs calcium to bone rather than arteries. It is a plausible hypothesis with limited human outcome evidence — interesting, not established, and not a reason to pay a premium.

What actually protects bone

Worth keeping in proportion, because calcium is the part everyone focuses on and it is not the strongest lever.

  1. Resistance and impact exercise. The only intervention that stimulates bone rather than slowing its loss — see strength training in menopause and starting from zero
  2. Balance training, because most fractures require a fall
  3. Enough protein, which is part of bone matrix and is under-eaten — see how much protein you need
  4. Adequate calcium and vitamin D — necessary, not sufficient

Where hormone therapy fits

Hormone therapy does prevent bone loss and reduce fracture risk — a demonstrated effect rather than a marketing claim. It is not prescribed for bone alone in most circumstances, and protection lasts only while you take it. See bone health in menopause and HRT in your 60s and beyond.

What to ask

"I'd like to know whether I actually need a calcium supplement. Could we check my vitamin D, and could you look at my dietary intake before deciding? If I do need one, what dose closes the gap rather than a standard tablet?"

That question is the difference between taking a supplement because everyone does and taking one because you need it.
